Making your appointment

To schedule an appointment, please call the office location you would like to visit. If you have any questions, or if you should need to cancel your appointment, please contact us as far in advance as possible.

Where to go

Our main office is located on the fourth floor of the Northland Medical Center at 10th Avenue East and First Street, across from St. Luke's Hospital (see map for details). There is a patient drop-off zone near the First Street entrance.

Parking is free in the adjacent Northland Medical Center parking ramp.

What to bring

To make your appointment go as smoothly as possible, we ask that you bring the following items, if applicable:

  • Insurance card: For proper billing of your visit, we will need to record information about your medical insurance from your insurance card.
  • Forms: We use a number of forms to collect information for your visit. Please download the appropriate forms to fill out at your leisure and bring to your appointment. If you prefer, you can fill out the forms at the office (in this case, please arrive for your appointment 15 minutes early).
  • X-rays: If you have had x-rays taken that relate to your current problem, please bring them, or have your doctor send them to our office. This will save you the time and expense of having new x-rays taken, and may avoid the possible rescheduling of your appointment.
