Privacy

We will always protect your privacy. Please see our Notice of Medical Information Privacy Rights for details.

Your medical records and x-rays are kept in the strictest confidence. They will be seen only by those directly involved with your care, and they cannot be disclosed to anyone without your written permission. To protect your privacy, we require that you sign an authorization form to arrange to have your records sent to or from Orthopaedic Associates.

Prescription Refills

When you need your prescription refilled, please contact your pharmacy during our regular office hours. Prescriptions cannot be filled after office hours or on weekends.

Identity Theft Protection

The Red Flag Rules are an Identity Theft Prevention Program designed to take reasonable measures to detect, prevent, and mitigate identity theft in connection with the opening of a covered account or any existing account. As a result, Orthopaedic Associates of Duluth, P.A. will now require the following information from all patients to ensure the identity of the person being seen:

1. Drivers License or state issued form of identification with picture
2. Proof of Insurance
